I couldn't find the journal article I mentioned, but I believe it was
related to Robert L Glass's presentation at Practical Software Quality
Techniques '99

Which in turn was based on his book "Software Runaways" (1997)
which had a chapter on CS90:
"2.3.4 Westpac Bank: The Anatomy of a Runaway"

[In hunting this down, I noticed the book was not favoured by reviewers
 so don't rush out and buy it just because I mention it, although it
 does have several disaster stories for Stewart.]
