I don't always do it myself, but does anyone take the time still to tell 
the management of these organisations that they are no longer accessible 
and have wasted their money so they can demand it back for 
non-performance?  One would assume that a working product would be a basic 
requirement of any contract.

Maybe that is the solution to the problem - get the designer/contractors to 
not get paid or have to do work over to eliminate the poor 
decisions.  Heck, make that awful/bad/terrible/dysfunctional decisions.
