Usually measured in characters, although sometimes in words (depends on the
text), and usually also affected by other factors (contrast, x-height,
leading, font face, etc).
Design is complicated to get right, and easy to get wrong. Sturgeon's law
applies. The preponderance of crap design doesn't invalidate the purpose or
aim of design, the same way the existence of a gazillion zombies on the net
doesn't invalidate the value of trying to make computers/networks more
secure ... unless maybe a couple of linker's would like to admit that the
work they do, and the causes they push, is just bogus snake oil too ;-)
