We do have a de facto fourth level of government below local 
government in Australia: the body corporate for an apartment building.

The apartment complex I live in has two levels of government of its 
own: there is a body corporate for each building and then a federated 
one to look after the shared land between the buildings. They have to 
worry about garbage collection, internal roads, street lights (solar 
powered), park, gardens, some power generation (reticulated solar hot 
water) and telecommunications (Cat5e to each apartment from the fibre 
node in the basement)

This is probably the level of government which differs least between 
countries and where broadband could make a difference. More 
generally, broadband will make a difference to the media, and thus to
