Wouldn't you just get the data from Births Deaths and Marriages?
After all, when a person is born or dies (at least in a Hospital or
in the presence of a medical practitioner) the practitioner has a
reporting obligation to BDM.
The relatives are left wide open to submit their forms when they
desire, within certain time frames of course. But BDMs already has
tentative data.
No need to collect from Doctors or Hospitals etc.
This is why we had so much hassle registering Delaney when she was
born two years ago. We had no medical records (never saw a Doctor,
never went to a hospital) I delivered her and it was just me and my wife.
