I can just imagine the crisis meetings in Boeing as the full disaster unfolded - the ritual shedding of blame & guilt onto others, the frantic combing through old documents for salvation or incriminating evidence...
It would be interesting to know the full story re the automatic ticketing system for Sydney public transport which I think has been canned by the State Government, and whether the original Requirements Specification was practicable in the first place. I can think of a wonderful example of the latter situation in local IT memory.
