Works fine with Mozilla Seamonkey and Firefox and Safari. In all
cases I could fill in the form at https://evisaforms.state.gov/ds156.asp
and then hit Continue to view the PDF that the site created from the
form data.

There is no need for their paranoid error checking, since the site
provides an HTML form (works in all browsers) and then offers the
result as a PDF that can be viewed stand-alone sans browser, printed
and saved.
