For carriers other than Telstra, adding ports / dslams is a trial of both
patientence and funding. Getting access to the exhanges requires a large
dollop of both resources, largely because it's in Telstra's interests to
make that process as difficult and expensive as possible. They can also
break out the old chestnut of "exchange full" even when it isn't for
"operational overhead".

Telstra itself has none of these issues.
