I'm certain that this is NOT a complete list (as it states in the  
preface).

I built and managed alia.org.au, and it was in existence through most  
of 1994, though we didn't launch it 'officially' until March 1995 -  
back in the days when sites were indeed launched. And that you could  
claim to be the first (in something no longer important).
