What typically happens is that an IP or IP range becomes a large enough
source of illegitimate looking traffic that the content providers rate limit
to subtly dissuade users connecting via there or just outright block it.

"Illegitimate" is hard to define but it's usually a combination of size and
concentration of traffic, owner of the IP range is not a normal access ISP
and lack of registered and/or premium users from that IP or range.
