I use http://www.xe.com/xetrade/ for transactions over $500.
$25 flat fee, and they seem to take about a 1.5% cut on the currency 
conversion.

For transactions under $500 I use Paypal which works out to be slightly 
cheaper and I also cover the cost of the PayPal fees. This doesn't work 
for sending a precise amount though, as the fees and currency conversion 
rates seem to vary.
