An oft quoted reason is that the politicians set ridiculous deadlines 
and so architecture/development/testing/implementation is conducted in 
panic mode.

In my experience it is very unfair to blame the IT department. The 
responsibility lies with general PS management (who agree to the 
deadlines) and politicians who operate on the election cycle.
