Yes, in my day we tested software 48 hours a day, working in paper bag,

In the court cases about failed systems I have been an expert witness 
for, the developers knew what they should have done, didn't do it, but 
hoped it would work out okay in the end. These were all settled out of 
court, because the corporate customers didn't want their lack of 
oversight becoming public.
