There is an alternative build-out model not discussed.

What's clear for Australia: there is no single technology/structure
suitable for all locations.
Who better to chose what's appropriate and acceptable than the community
that'll use it...
Not going to happen because it breaks the Telco monopoly.

I believe with high-gain antennas 802.11 wireless will do around 40km.
[Correction?]
There is already a good infrastructure of mobile phone towers around
that might be leveraged.
[platform + power + telco links]

I don't know if the towers could support the extra load of many extra
small antenna.
But these would only be used in remote locations with small user bases -
which seems a good fit.
