Trust. And a sensible system of security, legislation and penalties. As with with paper records.

I am fully aware of the risks of keeping information in a database, and the potential to screw things up royally, but the same applies with paper records, which were perfectly adequate in a time when people lived in the same place for years at a time and had the same doctor until their death, and when the likelihood of your attending an out-of-area hospital with a difficult problem were much more remote, and with records held by a doctor that you cannot remember.
