Each format is different. For generating PDFs with tables of contents, indexes I have found the best way is to create the document in openoffice and then use its builtin "export to PDF". Maybe you can do it with adobe but the integrated PDF export in open office works really well for that.
Epub has it's own kind-of built-in table of contents. I have published a book of poetry with a separate table of contents, an index and an index of first lines. It's tricky to get into epub but possible. You just have to do mess with the html a lot. A search facility is not the same as an index. Still haven't managed to get that one into the Apple store for other complex reasons - they have weird issues about titles and subtitles.
