Yep, I do to.  I ALWAYS create an email address based on the websites 
domain name and sometimes even a datecode.  Some organisation have multiple 
divisions that use different databases and it would, in my view, be unfair 
to penalise a department you have a good relationship, to that of one that 
did the wrong thing.  Plus it makes tracking good and when you complain to 
Good Department, it creates nice internal friction!

As to what I fill in, yep, I agree.  I'll put in what I feel is relevant 
and put in whatever I like in other "mandatory" fields that have no legal 
or relevant basis.
