Its absolutely trivial to implement - there is already perfectly  
adequate open source software to run surveys,
for example, limesurvey
so its just a matter of dropping it on a domain and running.
(It does use JS, but its not for core functionality, IIRC)

I'd do it, but my queue of 'cool stuff!' projects is already as long  
as my arm and I can't think how to make enough to cover its costs off  
the top of my head.
(I get in trouble for giving away too much of my time :( )
